Residential Home Health of NW OH, LLC,
located at 1745 Indian Wood Cir Ste 252, Maumee, OH 43537-4168,
will no longer participate in the Medicare program (Title XVIII of
the Social Security Act) effective October 31, 2026. The agreement
between Residential Home Health of NW OH, LLC and the Secretary of
Health and Human Services will be terminated on October 31, 2026,
in accordance with the provisions of the Social Security Act.
No payment will be made by the Medicare program under this agreement for covered services furnished to patients who are admitted on or after October 31, 2026.
